Case Management
The Community Support Program
The Community Support Program provides intensive case management supports to people with:
- Serious and persistent mental illness
- Concurrent Disorders (mental health and addictions)
- Complex Needs (dual diagnosis / acquired brain injury)
- Urgent Needs (needing immediate, non-crisis supports)
The role of case management is to assist people with mental illness to achieve independence – that is, the ability to live in the community with the least intervention from formal services and, to the greatest extent possible, to make their own decisions (including decisions about receiving case management services). Participation in the Community Support Program (or CSP) is voluntary, goal driven, and participant directed.
CSP services are provided in a flexible manner to meet a participant's changing wants, needs and resources. To the greatest degree possible, they are provided in the environment of your choice.
Case managers also help obtain required services by assisting with access to a full range of mental health and/or other community resources.
Case managers provide support to help individuals learn more about, and ultimately make decisions regarding:
- Monitoring of mental illness and symptoms
- Helping develop better coping strategies
- Developing and maintaining independence
- Education supports
- Employment / Volunteering supports
- Life and social skills development
- Health & wellness
- Linking with other community services
